The two girls, Lindsay and Jenny, try to escape from the demented surgeon, Dr. Heiter, but are quickly subdued and subjected to his twisted experiment. Heiter kidnaps a third victim, a Japanese man named Katsuro, and successfully completes the surgery to connect the three individuals into a human centipede, with each person's mouth sewn to the anus of the person in front of them.
As the three victims struggle to cope with their horrific situation, Dr. Heiter revels in his creation, forcing them to undergo degrading and humiliating acts as he trains them to behave like a single organism. Lindsay, Jenny, and Katsuro must find a way to escape before they become permanent fixtures in Dr. Heiter's twisted fantasy.
